Loss of Documents (Allied Health)

Cover for costs and expenses incurred in replacing or restoring documents (including patient records, clinical notes, treatment plans, and electronically stored health data) for which the Insured is legally responsible, where such documents are lost, damaged, or destroyed while in the Insured's custody or control. Typically requires: discovery during the Period of Insurance; notification within a specified timeframe (often 30 days); and substantiation of costs with invoices and accounts. Generally excludes: loss from computer viruses or data theft by former employees; wear and tear or gradual deterioration; and consequential losses. Cover is usually subject to a sub-limit.
